Where and When to Register for Grades K-12
Registration for the current school year is ongoing. Please call the Registration Office directly at 215-643-8971 for an appointment. All appointments are held at the Administration Building located at 1580 Ft. Washington Avenue, Maple Glen. Days/Hours: Tuesday – Friday, 8:00 a.m. – 3:00 p.m. (closed noon – 1:00 p.m. for lunch).

Information needed for Registration

1. Proof of Residency (Please supply one)

    • Copy of settlement paper, deed or Upper Dublin School or Township tax bill
    • Copy of rental lease
    • Copy of agreement awarding custody to resident of Upper Dublin Township
    • Notarized multiple occupancy affidavit
    • Notarized Section 1302 affidavit

2. Proof of Birth (please supply one)

Student entering Kindergarten in September must be 5 years of age by September 15th.

    • Birth Certificate
    • Baptismal Certificate
    • Passport

3. Proof of Immunizations (supply all dates with doctor’s signature):

    • 4 doses of tetanus ( 1 dose on or after the 4th birthday)
    • 4 doses diphtheria (1 does on or after the 4th birthday)
    • 3 doses of polio
    • 2 doses of mumps
    • 2 doses measles
    • 1 dose of rubella
    • 3 doses of hepatitis B
    • 2 does of varicella vaccine or history of disease

Children attending 7th grade need the following:

    • 1 dose of tetanus, diphtheria, acelluar perussis (Tdap) (if 5 yeats have elapsed since last tentanus immunization)
    • 1 does of meningcoccal conjugate vaccine (MCV)

4.  Need Copies of Report Cards, IEP’s and Evaluations (if applicable)

5. Parent/Guardian Identification Required
At the time of your appointment you will be asked to provide proper ID before your child is registered (i.e.   driver’s license, voter’s registration, employment badge, passport, etc.)

New & Kindergarten Registration:

Please note that in addition to proof of immunizations, Pennsylvania School Health Law requires that a private dental and private physical examination form be completed and submitted to the school health office in September of the year your child enters school. Helpful hint:  Providing the physical exam form at the time you request your child’s immunization records from your pediatrician will eliminate being charged a form fee two times by your pediatrician office.
